OBSERVATIONS 25 DCMR CORRECTIVE ACTIONS
10. - There is no handwashing signage at the handwashing sink located at the sushi bar.  3004.1 A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visibly to food employees.  
29. - The establishment is not following its HACCP plan. There is no indication of monitoring or verification of temperatures. There is no indication of monitoring or verification on the pH logs. During the visit no one can demonstrate Ph. meter and thermometer calibration. There is no distilled water. There is no PH calibrations solutions during the visit.  4104.2(a) If the Department grants a variance as specified in Section 4102, or a HACCP Plan is otherwise required as specified in Section 4202, the licensee shall: (a) Comply with the HACCP Plan and procedures submitted and approved as the basis for the variance. 38. - The date that pest extermination services were last provided to the establishment is not available.  3210.2 The licensee shall maintain a copy of the establishment's professional service contract and service schedule, and the following documents shall be available for inspection: Pf (a) Name and address of its District-licensed Pesticide Operator/contractor; (b) Frequency of pest extermination services provided under the contract; and (c) Date pest extermination services were last provided to the establishment.  
56. - Observed grease and dust accumulated at the cooking ventilation hood.  1509.4 Hoods, grease-removal devices, fans, ducts and other appurtenances shall be cleaned at intervals necessary to prevent the accumulation of grease.

CEASE AND DESIST- You are hereby ordered to Cease & Desist operation of Acidification of white rice EFFECTIVE IMMEDIATELY. Please contact the main office to speak with an area supervisor at (202)535-2180 or email the area supervisor at food.safety@dc.gov for further direction.

The licensee has failed to comply with the 5 calendar-day notice cited on 6/05/2021, as listed within the observations section of this report. The Licensee’s failure to comply may result in additional enforcement action.

5 calendar day notices have been corrected from the previous inspection conducted on 6/22/2021.[25-A DCMR 203.3, 25-A DCMR 904.2, 25-A DCMR 802.1(a)-(b), 25-A DCMR 803.1, 25-A DCMR 1007.1, 25-A DCMR 816.1, 25-A DCMR 812.2, 25-A DCMR 1809.2 and 25-A DCMR 3201.1.]
